The Care Program DifferenceSilverado At Home specializes in create care programs for seniors, enabling them to remain living in their residence, be that home or senior care facility. The Silverado At Home Care Program is so different because of the clinical care component.
Beginning with a thorough and professional geriatric care needs assessment, the entire case is administered by a clinical Geriatric Care Manager, typically a Registered Nurse or Masters Social Worker. The Care Manager leads the Home Care Program by developing a care plan unique to each client, implementing this plan with the Care Companions, and being on call for any unforeseen needs.
Care Companions all are given thorough background checks, and undergo training at one of the Silverado Senior Living communities to establish an understanding of dementia care. Though qualified for any care needs, Silverado At Home does specialize in handling Alzheimer's and other memory care needs. Care Companions are available for hourly (four hour minimum), overnight, and live-in positions.
