An integrated model of care

Silverado At Home has combined the concepts of superior care management provided by geriatric professionals with in-home companion care services. This model promotes the highest quality of care delivery by creating regular interactions between care companions and care managers. An integrated model of care promotes high quality service delivery by providing an expert resource (geriatric care manager) available for regular consultation with our clients and with care companions in the home. This enhances communication and allows for rapid response to changing care needs of those we serve.
 

How does an integrated Care Model work?

  • It begins with a thorough assessment performed by a geriatric care manager.

  • The results from the assessment process are shared during a service plan meeting
    attended by all interested parties.

  • Recommendations are made for care options that meet the specific needs of the client.

  • The client and their loved ones make choices that best match their needs.

  • A care manager is assigned to provide care oversight and if desired, a home care companion is matched to serve the unique needs of the client.

  • In cases that involve a home care companion, the companion will work under the guidance and supervision of the assigned care manager, promoting high quality service delivery.

Comprehensive Assessment
A comprehensive assessment is used to initiate new client cases. It is essential for our care managers to understand our clients and their family’s needs. A professional geriatric care manager administers the assessment in a client’s current living setting including private homes, independent living settings, assisted living settings, or even hospital settings. The assessment takes approximately 2-3 hours to administer. The assessment explores psychological, cognitive, behavioral, socio-cultural, and physical functioning. Additionally, the living environment is evaluated for safety and appropriateness.

Results and recommendations are presented in a written report that often may serve as a life plan as memory and/or health decline. The assessment and interpretation process allow us to develop a service plan that has the greatest likelihood of enhancing quality of life.

Geriatric Care Management
A geriatric care manager is a professional trained to assist with a variety of senior care needs. Typically, geriatric care managers are master’s level social workers or licensed nurses. Silverado’s geriatric care managers are active members of the National Association of Professional Geriatric Care Managers, the premier professional care management organization. Examples of services performed by care managers include the following:

  • Comprehensive Assessment and Follow-up Recommendations

  • Ongoing management of care needs (Changes in condition)

  • Consultation with other professionals

  • 24 hour access (on-call)

  • Long-term planning including regular communication with families

  • Placement or other care decision-making assistance

Home Care Companions
Silverado’s home care companions are carefully selected, trained, and matched to the unique needs of each client. Each care companion is insured and has the capacity to transport clients on outings or to appointments. Prior to beginning work for Silverado, all care companions must pass a thorough criminal background check and a physical. Before being given an assignment, care companions complete orientation and training specific to meeting the needs of seniors with memory care concerns. Each care companion must then spend a full shift shadowing an existing care companion in one of Silverado’s assisted living settings. This allows us to observe and carefully evaluate performance before sending an individual to a private home setting. This investment in our care companions separates Silverado from most other home care providers. Care companions are available for live-in or hourly assignments.

Silverado At Home combines professional care management with uniquely trained care companions. The result is a bright, reliable staff that is exceptional at engaging seniors with memory care needs in any setting. The Silverado model promotes the highest quality of care delivery by creating regular interactions between care companions and care managers.

The development of Silverado At Home was a response to requests from our customers to provide as many options as possible for people seeking access to Silverado’s memory care expertise. The relationship with our AL communities gives us great training and staffing advantages. All of our Silverado At Home care companions spend at least a full day in our Silverado AL communities interacting with a diverse range of individuals with memory care needs including those with Alzheimer’s and other dementias. This experience allows us to see our care companions in action before sending them out into a home setting. It is the most important part of our training to ensure that we have the best care companions in the industry. Most other home care providers simply do not have the advantage of delivering this kind of training.
Reliability is always a question in the “home care” industry. Silverado is able to provide great reliability because of our large caregiver pools and affiliation with our assisted living communities. This allows for many back-up plan options if a care companion becomes ill.

Our Clients

The following are examples of the types of clients that are served by Silverado At Home:

  • An 82 year old man with moderate dementia was hospitalized following a hip fracture. A care companion was hired to stay with him during his hospitalization so that he would not place weight on his hip and jeopardize the healing process.

  • A 73 year old female was living at home alone and was recently diagnosed with earl Alzheimer’s disease. Following a comprehensive assessment, a care companion provided assistance 5 hours per day during weekdays while her daughter was at work. A geriatric care manager helped arrange additional support services for both the daughter and the client.

  • A live-in care companion was hired to stay with a frail 93 year old female while her son and his wife went on a two week vacation to Europe.

  • The family of a an 84 year old man hired a care manager to meet with his family a couple of times per month to assist them with the planning process relative to their father’s care needs. He is beginning to have some mild memory problems and they want to be prepared if further changes occur.

  • A comprehensive assessment was requested for a 68 year old female living in an independent living setting. She had begun to display some unusual behaviors and her family wanted some assistance with understanding their mother’s behaviors.

  • The spouse of a 76 year old male with moderate to late Alzheimer’s disease requested help with her husband due to her declining health. After an initial assessment, daily companion care was scheduled as well as ongoing care management. The well spouse began to show improvement in her own health.

  • A Geriatric Care Manger was hired specifically to assist a family with the process of finding an assisted living environment that would be most conducive to their mother’s level of functioning


 

FAQ
What is the process for obtaining service through Silverado At Home?
Most new clients begin by receiving a comprehensive assessment in the home setting that covers all aspects of functioning as well as safety considerations within the home. This process takes approximately 2-3 hours and is conducted by a geriatric care manger (usually an MSW or RN). The results of the assessment are used to develop a service plan with the client. The service plan usually involves some level of home care accompanied by some level of geriatric care management services.

How do you promote quality care in private home setting?
All clients receiving home care service are assigned a professional geriatric care manager to monitor the care plan through regular communication with care companions and families. This arrangement provides a real resource for care companions as questions arise. It also provides immediate support for family members who may have changing perceptions about care needs and services.

Will long term care insurance cover home care and/or care management?
Policies need to be reviewed on an individual basis. Most polices provide a daily rate of reimbursement for services that covers at home care. It is becoming more common for LTC insurance to have additional coverage for care management built into policies.

What are your hours of availability?
Silverado At Home has a 24 hour on-call service that allows for rapid response to service requests. This includes weekends, evenings, and holiday.
